ALS United Rocky Mountain collaborates with some of the best ALS physicians and clinics across Colorado, Utah & Wyoming to help ensure that people living with ALS have access to specialized care, based on best practices. ALS United Rocky Mountain Clinics and Centers provide compassionate care in a supportive, family-oriented atmosphere. 

Multidisciplinary Care

Our multidisciplinary care model brings together a team of health care professionals specially trained to address the needs of people living with ALS, allowing them to receive care from each discipline during a single visit. The care team typically includes a neurologist, physical therapist, occupational therapist, nurse, dietitian, speech language pathologist, social worker, respiratory therapist and an ALS United Rocky Mountain liaison.

By attending Kaiser Permanente’s ALS multidisciplinary clinic appointments, our Care Coordinator serves as an advocate and guide for patients and families. They help translate medical information into practical support, ensuring families understand their options and feel empowered throughout their ALS journey. We encourage patients and caregivers to arrive with questions or concerns they would like addressed. Your ALS United Rocky Mountain Care Coordinator will be present to help guide conversations and ensure you leave with a clear plan of care and support.

Their presence helps bridge medical care with daily living needs, ensuring comprehensive, coordinated support beyond the clinic walls. During your clinic visit, you will meet with each clinician in one coordinated appointment. Our ALS United Rocky Mountain Care Coordinator will be available to answer questions, help prioritize next steps, and follow up after your visit with resources and referrals.
